How do I join?

1) Sign up online or book a free Meet and Greet first (links below)

 

2) After signing up, you can schedule your first appointment

 

3) Before your appointment Dr. Adamchak will have you complete a consent and medical record release so that she can obtain your past medical records.

 

4) You will also receive a health history questionnaire to complete at your leisure. Or don't! and we can review it all in person- we'll have time!
