What is Direct Primary Care?
Direct Primary Care (DPC) is a healthcare model where members have direct access to your primary care doctor in exchange for a flat monthly fee.
​
This means Dr. Adamchak is your doctor, all the time, with greater availability than traditional fee for service offices can provide, thanks to a smaller patient panel size and reduced burden from insurance requirements.

My Story
Originally from East Otto, I am returning home with a wide range of experiences and a people-first approach to practice family medicine. Growing up in this rural farming community fostered a down-to-earth perspective and has shaped the way I care for my patients. After graduating from Ellicottville Central in 2007, I went on to study at Case Western Reserve University in Cleveland, where I earned my bachelor’s degree in Biology and Spanish—and played soccer for the Spartans along the way. Upon graduation, I headed back to New York to attend medical school at SUNY Upstate Medical University in Syracuse through the US Army's Health Professions Scholarship Program.

After graduating medical school in 2015, I completed my Family Medicine residency at Madigan Army Medical Center at Joint Base Lewis-McChord. I also met my husband Joe while there in Tacoma. My first duty station brought us to Stuttgart, Germany, where I cared for service members, their families, and retirees in an outpatient clinic. During that time, I also completed a short deployment to Kabul, Afghanistan, which added to my experience working in fast-paced and challenging environments. I continue to serve as an officer in a Medical Readiness Unit with the U.S. Army Reserves.

After fulfilling my active-duty commitment, my family and I returned stateside and we landed in Sneads Ferry, NC where Joe could continue his work with the Special Operations community. I spent three years practicing at MedFirst Family Medicine and Urgent Care. We’ve now made our way back home to Ellicottville, where we’re excited to raise our three boys in this special community that means so much to me. Outside of work, I love spending time with my husband and children, staying active through exercise and sports, and traveling whenever we can.
I am board-certified in Family Medicine and believe strongly in building genuine relationships with my patients, listening closely, and providing care that’s thoughtful, evidence-based, and tailored to each individual. I look forward to getting to know you and partnering with you and your family on your health journey.
